Unfortunately, it is just a random function. All the details about failures are
decided at the start of a race. This means that when you restart from a saved
game (saved at the start of the race) after a failure in, say, lap 30, you'll
encounter the same failure in exactly the same place over and over again!
You shouldn't restart anyway, BTW :-)
