You are your own worst enemy, I believe -- by hosting on a server on
your LAN, and then going through an Internet connection (even if it is
a relatively-fast cable modem), you're likely to suffer more lag than
if you stood up the server on your own machine and then played on the
same machine that's hosting the server (I've done this with great
success on a 56K analog modem connection).
You might also need to "tune-up" your networking settings -- I'm not
very well-versed with the nuances of tuning a cable modem connection,
but I recommend that you start here to learn more about tuning online
connections in general:

Before you bend over backwards tweaking and tuning your connection,
though, I'd recommend trying my first suggestion to see if that helps
(hosting the server on the machine you're playing on).
