If your inside tires are low enough on the apron when you come around by the
pit entrance, sometimes it will flag you, but I've never had this happen to me
when I was driving near an optimum line.
Also, once you stop and go, you cannot travel over 55 until you leave the pit
lane or you will be flagged again.
Also, do not come barreling into the pits for a stop and go. If you enter the
pits at above 55, even if it's to serve the penalty, you will be swiftly
awarded another.

It sounds like you are exceeding the pit speed limit during the yellow
flag stop. Remember, the black flag only comes out once the race
returns to the green. You are not being flagged on the lap you see
it but from the infringement maybe 3 or 4 laps before.
