I don't have a form anymore (sent it in) but I recall the jan 31 date.
Try e:mailing sierra, or their forum.

The rebate covers Sierra Sports games purchased between 10/1/98 and
1/31/99 - rebate requests must reach Sierra by 2/28/99.

I sent mine in November and never received it...
I recall seeing January 31, 1999 as the cut-off date.

I called the 1-800 number on the rebate slip. The guy told me they had
just received my rebate request and that it was sent to "processing". I
asked him how much more time it would take. He told me "6 to 8 weeks!".
It's been 13 weeks already! How much time does it take to make a ***y
cheque?!?

Sent mine in in November (2 or 3 days after I got it) with MS Golf 1.0
and got my rebate check back a week or so ago! They're still mailing 'em,
hang in there.

You have to wait for them to get their Tax Refund Check :)

Seriously, they are in NO hurry to give you money. The fact that they
eventually do keeps them legal...
