Yes you do. The instructions are in the ReadMe3D.txt at the papy site:
www.papy.com

I have a V3 card, I simply added the lines to the bottom of my core.ini
file, made the recommended changes to suit my card and it fired up just
fine.

Technically you don't need to add anything to the CORE.INI if you just
want to use all of the default values, however if you want to edit
anything you'll need the entire [Int Profile Values] section as well as
the [Direct3D7] section (these can be cut and pasted from the
readmeD3D.txt).

Didn't you have that problem:

When I use the new patch and set the core.ini settings to the values,
papy told us, I have very good framerate and marvellous pictures, but
over and under the game-screen, in the black aera, the are building up
more and more bright groups of pixels. Seems so, as if they are copying
out from the game-screen, sometimes, there is a little flicker in parts
of the screen.

Hi guys
With the new D3D patch, do I have to change the core.ini file or anything
else?
I've seen people posting codes up here. Do I have to change to those? Please
enlighten me, thanks.
