From what I hear, in GPL there are race modes where you can recover from
accident or failure (using Shift-R) and modes where you can't.

In the cases where you can't use Shift-R, what are the consequences of a
smash or failure during qualifying ?

Return to the pits and take another car out. Simple really. It just means you
can't hurtle through a slow corner and gain an advantage by jumping back on
the track when the accident stops and make a fast lap.
